How a alarm works
A mechanical alarm watch has a second mainspring that powers a small hammer. The wearer sets an alarm hand to the wake time, and when the hour hand reaches it, the hammer strikes the inside of the case back. The sound is a buzz or a tone rather than a bell. The alarm has its own winding crown, and it sounds until it runs out of power or is switched off.
